Easy Application Process

Thank you for your interest in selling Earthy Meds products. In order to help us evaluate your request for reseller pricing, please fill out the application below completely. Once you submit the form, a member of our Sales Team will reach out to you.

To complete the application, you’ll need to download and fill out this form: E-595E Streamlined Sales and Use Tax Agreement Certificate of Exemption. You’ll also need a copy of your liability insurance policy.

As you fill out the form, please remember to: 

1. Add your business name for Name of Purchaser. 
2. Add your business address.
3. Add your business tax ID number or FEIN (Federal Employer Identification Number).
4. Check the appropriate option under Type of Business. For example, most of our partners use 10. Retail trade.
5. Check the appropriate option under Reason for Exemption. For example, most of our partners use G. Resale (include your state tax identification number if applicable).
6. Use our address for Seller’s Address: Earthy Meds, 825 C Merrimon Ave. #213, Asheville, NC 28804.
7. Sign the form.
